Establish Care In Office Appointment $250

Following the completion of your new-client intake form when scheduling, I will review this prior to our appointment. This will allow us to progress further and will provide more effective use of your scheduled time. During your visit we will focus on gathering as much relevant information about your health as possible. A thorough look at health history, medical history and diet/lifestyle information helps to provide valuable insights that will allow us to help you more.

Establish Care Long Distance Appointment $250

30 minutes

Schedule phone appointment call for two weeks out. Following the completion of intake form when scheduling, directions will follow in confirmation email on how to send a DNA sample to the office. After receiving your sample, we will email your reports and connect over the phone.

12 & Under

12 & Under Establish Care In Office Appointment $125

1 hour

Booked 4-6 weeks after your initial consultation, it is essential to monitor & discuss your ongoing treatment plan. Depending on how complex your health concerns are, multiple appointments scheduled over a varying number of weeks/months may be necessary.
